Join us as we hear from Dr. Tonya Van Hee, Hospice Medical Director at Park Nicollet as she discusses Hospice and Palliative care. Dr. Van Hee will be joined by Program Director Jenn Faris, Dr. Jane Oh, and Hospice Social Worker, Deb Schmidt. Together, they will be speaking about the difference and progression from palliative care to hospice services and will review what services each program will provide. They will also briefly discuss disease progression trajectory and how palliative care can be a “value add” for prospective patients.
